Application forms may be obtained from any credit union in BC. Only original forms accepted; no photocopies. Please direct all other enquiries in writing to the above addresses.

FOR CREDIT UNION STAFF:
Credit unions may request a package of application forms and other printed material by emailing cufoundation@shaw.ca. Provide the name of your credit union, branch, transit no. and to whom the package should be marked attention.

Revision dates are shown at the bottom of the form. The most recent printing is REVISED 2008. All printings prior to 2006 should be destroyed as they will not be accepted.
